| THE APPLICATION FORM DOES NOT LOOK RIGHT ON MY SCREEN(overlapping drop down boxes, long rows over the input fields, etc.) WHAT IS GOING ON? First, you should let the ENTIRE form load BEFORE you scroll. It is a big form. In short, your computer does not have enough memory (RAM) available to render the form correctly. Here is the solution that will work in most cases: 1. Clear the cache (temporary files) in Internet Explorer (Usually Tools>Internet Options>Remove Temporary files 2. Shut down Internet Explorer 3. Restart the computer 4. Open JUST MS Internet Explorer. 5. Go to Web site, fill out form. In most cases, this will solve the problem.... |

| WHEN AND WHERE IS REGISTRATION FOR BOTH THE GIRLS AND BOYS TOURNAMENTS? Team Registration for ALL teams will be held at the Larkin House located at the tournament site. Directions can be found on the field direction link. Girls Tournament Registration: will be held Friday June 13th from 5:30PM till 8:00PM. Boys Tournament Registration: will be held Friday July 11th from 5:30PM till 8:00PM. There will be NO registration at the fields. PLAYERS AND /OR TEAMS WHICH ARE NOT COMPLIENT WITH REGISTRATION REQUIRMENTS WILL NOT BE APBLE TO PARTICIPATE AND WILL FORFEIT THE TOURNAMENT FEE. |

Player Books or player cards (for each player and guest player) 3. Travel Permit (if from outside of EODSA district) 4. Guest Player Roster (if applicable) |
| I HAVE MISPLACED MY ORIGINAL REGISTRATION FORMS. HOW CAN I GET DUPLICATES? Downloadable versions of the form in Word and/or PDF file format were made available to you when your team was accepted and you confirmed your participation. If you need a duplicate form and have already confirmed, you will need your TEAM ID Number provided to you to access the forms. In the tournament website, http://www.osusoccer.com log in with your TEAM ID Number, and the system will tell you that your team has already been confirmed. There will be a link to the page that contains duplicate forms. We recommend the use of the Word file format. |
| WHEN IS MY TEAM ROSTER FROZEN? Team roster is frozen at the Friday night team registration. |
| WHERE DO I SEND MY TEAM ROSTER AND TRAVEL PERMIT? 1.Travel permit is due one week prior to the tourmnament and must be faxed to 613-692-0489. It is a good idea to also bring a copy of the travle permit with you at the team registration on Friday night. 2.Final team roster is due when the team registers for the tournament the Friday night. |
